Parent Permission Forms

Parent Permission Forms for 2010-2011 are available through the "FORMS" page above or at http://peninsulafmc.org/forms/PermissionForm10.pdf. Please fill one out and turn them in to one of the youth group leaders. We will require them for all youth group events!
